This form documents the remaining requirements that must be completed the summer semester following the commencement ceremony. This is required of any student who is participating in commencement without having all degree requirements met.

Have the instructor sign next to the class added/dropped.

Submit it to the registrar's office.

Please have your division chair or program director sign this form prior to submitting it to the registrar's office.

This form is intended for students who plan to leave Spring Hill College between semesters. This can be with or without the intent to return to Spring Hill College.

This form is intended for students who wish to withdraw from all courses after the start of the term for which they are enrolled.

This "Official Transcript Request" form is for hard copies only. If you attended Spring Hill College prior to 1990, you may have to request your transcript from the Registrar's Office using this form.

Official Transcripts should be ordered through the National Student Clearinghouse. They will be sent electronically to the receiving institution or company.
